Holistic Mind Therapy was founded with the goal to help individuals live a more fulfilling life. Therapists at HMT create a non- judgmental environment and follow a wide range of techniques in the counselling and therapy sessions, to equip the clients with life-long personalized techniques that can help them navigate through life in a healthy and wholesome manner. Please read this important information about our services and policies carefully. If you decide to avail our counselling or/and therapy services, please read and sign this consent form.

Counselling at Holistic Mind Therapy
  • We offer face-to-face counselling as well as online sessions.
  • One session is for 45-60 minutes.
  • The fee for the counselling or therapy session is to be paid before taking the session.
  • The first 2-4 sessions will involve a comprehensive evaluation of your needs. Counselling and Therapy do not always guarantee a positive outcome as the results vary from person to person and achieving a positive outcome depends upon the client’s willingness and efforts to change. You are, however, free to terminate the service at any given point, if unsatisfied.
  • Due to the nature of services involved, we provide no refund (in case of termination of services either by the client or the therapist) for any of the programs you are enrolled in with us.
  • The counsellor/ therapist will keep the information you share confidential, unless they have your written consent to disclose certain information.
  • Please note that to better meet your needs, your counselor/ therapist may refer you to conduct a psychometric assessment.
  • If you are receiving treatment or counseling from other medical facilities or services, your counselor may contact your doctor or person in charge with your consent.
  • We do not prescribe medication, but if requested or required as per your counsellor/ therapist’s opinion, we may refer you to another health care professional depending on the severity of your condition.

Confidentiality

1. Information discussed during your therapy session and all documentation is kept private and confidential.
Counselors don't disclose your information without your consent. Some very important exceptions to this rule are:

  • (1) If there is a legal responsibility for the therapist to release information contained in records.
  • (2) The fee for the counselling or therapy session is to be paid before taking the session.

2. Counselors may present your case at professional conferences or study groups in order to enhance their skills and knowledge. Your personal information will be de-identified to protect your confidentiality.

Your Rights and Responsibilities as a Client

Rights

  • You have the right to ask questions about anything that happens in therapy.
  • You have the right to considerate, safe and respectful care, without discrimination as to race, ethnicity, color, gender, sexual orientation, age, religion, national origin, or source of payment.
  • You have the right to ask questions about any aspects of therapy and any specific training and experience of your counsellor/therapist.
  • You have the right to expect that your counsellor/ therapist will not or try to have social or sexual relationship with you during or after you are no longer a client.

Responsibilities

  • You are responsible to make an advance payment for your session unless we have made other firm arrangements.
  • You are responsible to come for the session on schedule time, if you are late, we will end on time and not run over into the next client’s session.
  • Your relationship with your counsellor/ therapist is solely for availed therapeutic treatment and a professional relationship.
  • You will not have any inappropriate or try to have a sexual relationship with your counsellor/ therapist.
  • If you violate our policies, the counselor/ therapist may terminate the service.
